Job Summary
The Case Manager is responsible for coordinating and overseeing services for individuals in accordance with New Jersey Division of Developmental Disabilities (DDD) guidelines. This role ensures services are delivered effectively, remain compliant, and support the health, safety, and quality of life of each individual.
Case Managers serve as the primary point of contact for assigned individuals and manage all aspects of their caseload, including service coordination, staffing, compliance, and communication.
Key Responsibilities
Service Coordination & Oversight:
Implement and monitor Individualized Service Plans (ISPs)
Ensure services align with DDD authorizations and individual needs
Maintain consistent communication with individuals, families/guardians, and Support Coordinators
Address service gaps, changes in needs, and concerns promptly
Caseload Management:
Manage both
active cases
(ongoing services) and
staffing/pending cases
(new referrals)
Serve as the primary point of contact for all assigned cases
Ensure continuity of care and service delivery
Referral & Intake Coordination:
Process new referrals from intake through service initiation
Collect and maintain required documentation (ISP, PCPT, SDR, AENF, etc.)
Coordinate onboarding and timely start of services
Staffing & Scheduling Oversight:
Assign and coordinate Direct Support Professionals (DSPs)
Manage call‑outs, coverage, and re‑staffing needs
Maintain scheduling and tracking through agency systems
Ensure staffing stability and service continuity
Communication & Quality Assurance:
Maintain strong communication with all stakeholders
Promote person-centered practices and community inclusion
Identify and escalate safety or compliance concerns
Regulatory Requirements:
Follow all
DDD guidelines , Medicaid waiver programs, and agency policies
Maintain HIPAA compliance and protect confidential information
Qualifications
Experience in case management, healthcare, or human services preferred
Knowledge of DDD services and IDD population is a plus
Strong organizational, communication, and problem-solving skills
Ability to manage multiple cases and deadlines
Proficiency in documentation and compliance tracking
