Job Summary
Advance Practice Provider in Rapid Response Team for adult medical patients.
Responsibilities
Participates in the Rapid Response Team, collaborating with medical and bedside nursing staff to respond to acute changes in patient status.
Performs comprehensive health assessments and manages patients with acute health problems.
Obtains patient histories and conducts physical examinations.
Writes orders and progress notes as indicated.
Assesses and manages common health problems.
Orders and evaluates relevant diagnostic tests, including radiology, laboratories, and ECGs.
Prescribes pharmacologic agents and nonpharmacologic treatments for appropriate medical conditions.
Educates patients and families regarding disease processes, medical interventions, and health promotion.
Documents patient teaching and discharge summaries as needed.
Refers and communicates pertinent abnormalities to the primary medical team.
Serves as a resource for nursing and ancillary staff regarding patient and disease management.
Addresses any clinical patient care issues, concerns, and follow‑up as needed.
Actively participates in sepsis reduction program.
Performs other duties as assigned.
Requirements, Licenses, and Certifications
Critical care experience preferred.
Licensed Registered Nurse in New Jersey.
Certified Nurse Practitioner in New Jersey.
Master’s degree in Nursing from an accredited program.
CDS, DEA, BLS, ACLS certified.
Essential Functions
Assists the Physician in the office with taking history and physical, administering medications, performing physical exams, and participating in patient flow through the practice.
Assists physicians with various procedures and surgeries and coordinates patient cases.
Organizes and achieves quality performance in routine care.
Coordinately aspects of patient care within a surgical health environment.
Demonstrates strong organizational skills, effective communication, and time management.
Prioritizes work assignments and demonstrates effective communication skills.
Implements policies and procedures, performs assessment and reassessment of the patient according to department standards and identified tools.
Performs other duties as assigned by the Physician.
Compassionately interacts with patients across different care areas.
Recognizes areas for improvement in divisional policies or patient care, initiates change, and educates all involved.
Communicates regularly with nursing leadership and staff regarding patient care issues.
Explains procedures to patients prior to testing.
Provides appropriate patient teaching as necessary.
