Licensed Practical Nurse - LPN

We are seeking Licensed Practical Nurses (LPNs) to provide nursing care in a Skilled Nursing and Behavioral Health environment. LPNs will administer medications, monitor patient conditions, and work closely with the healthcare team to deliver quality care.
Responsibilities
Administer medications and treatments as ordered
Monitor residents and report changes in condition
Document care and treatments accurately
Assist with patient care planning
Follow facility policies and regulatory standards
Requirements
Active LPN license
Skilled Nursing and/or Behavioral Health experience preferred
Ability to work assigned shifts, including weekends if required
Strong clinical and communication skills
