We are seeking a highly organized and detail-oriented

Coordinator, Central Intake Case Manager

to join our healthcare operations team in Atlanta. This role is pivotal in ensuring that complex case management referrals are processed accurately and routed to the appropriate clinical teams.
This is an excellent entry point for a professional looking to establish a long-term career within a premier healthcare organization. While initially a 3-month contract, there is a strong possibility for extensions, and contract workers are encouraged to apply for permanent (FTE) positions as they become available.
Status:

Full-Time Contract (3 Months with potential for extension/FTE conversion)
Schedule:

Mon-Fri, 8:30 AM – 5:00 PM
Work Environment:

100% Onsite (Non-Patient Access Building)
Key Responsibilities
Referral Management:

Process and assign complex case management referrals within the HealthConnect/EPIC system.
Workflow Coordination:

Ensure timely distribution of cases to the Case Management Team to maintain high standards of patient care coordination.
Data Integrity:

Utilize analytical skills to verify information and maintain accurate electronic health records.
Required Qualifications
Education:

High School Diploma or GED required.
Technical Skills:

Functional knowledge of computers and basic software applications.
Soft Skills:

Strong written and verbal communication, analytical problem-solving, and the ability to build effective working relationships with multidisciplinary teams.
Preferred Qualifications
Experience:

Minimum of six (6) years of experience in a healthcare setting is highly preferred.
Systems:

Proficiency in

EPIC , Microsoft Teams, Word, Excel, and PowerPoint.
Certifications:

Completion of courses in ICD-9/ICD-10, CPT coding, and Medical Terminology.
Assessment:

Demonstrated customer service aptitude (Customer Service Assessment).

This position requires 0% travel.
Patient Interaction:

This role is located in a professional administrative office; you will spend less than 50% of your time in a patient access building.
