Overview
Home Health & Hospice Care (HHHC) is one of the oldest Visiting Nurse Agencies in the country, based in Nashua, NH, supporting 25 communities in Southern NH. We specialize in homecare and hospice care and operate a 16‑bed Community Hospice House.
We seek registered nurses (RN) with 1 year of acute care med/surg or acute rehab experience. The position offers a $12,500 sign‑on bonus.

Responsibilities
Complete an initial home health assessment of patient and family to determine home care needs.
Provide a complete physical assessment and history of current and previous illness(es).
Utilize the nursing process in planning, providing, and evaluating skilled needs.
Complete a comprehensive patient assessment reflected in accurate OASIS scoring.
Develop a care plan establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in planning and revising.
Case manage patients, helping them recover and feel strong and healthy.
Qualifications
Graduate of an accredited school of nursing or hold an associate degree in nursing.
Registered RN with the New Hampshire Board of Nursing.
One year of recent acute care experience, preferably in med/surg or acute rehab.
Ability to exercise initiative and independent judgment.
Preferred Qualifications
Bachelor’s degree in nursing.
Community Health/Education experience.
Certifications: IV, Chemotherapy, Ostomy, etc.
Familiarity with OASIS and reimbursement drivers.
EMR software proficiency.
Required
Registered RN with New Hampshire Board of Nursing.
CPR Certification.
Satisfactory criminal record.
Satisfactory motor vehicle record.
