The North Central Health District’s Perinatal Health Home Visiting Program is a warm, community‑centered initiative designed to bring high‑quality healthcare, education, and support directly to families in the rural areas of our district. Our team works closely with expectant and new parents, helping them navigate pregnancy, birth, and early childhood with confidence. By meeting families where they are, we build meaningful relationships, reduce barriers to care, and make a real difference in maternal and child health outcomes. Under supervision, provides professional nursing care in accordance with nurse protocols, policies, and procedures, while playing a critical role in promoting maternal and child health, reducing disparities in birth outcomes, and supporting overall community wellness.

Primary Duties & Responsibilities

Focuses on the prevention of illness, injury, or disability, the promotion of health and maintenance of the health of populations.

Cares for populations with special needs during disasters/emergencies and biological and chemical protocols.

Provide home visiting services to identify, assess and provide care coordination services to eligible perinatal patients and qualifying infants.

Coordinates program services with other service providers and community organizations to promote optimal implementation.

Screen and refer clients to other community service providers.

Monitoring participants according to program guidelines and providing education and materials that promote wellness, safety, and awareness of pregnancy and postpartum complications.

Monitor for infant complications to include feeding, weight, head circumference, and developmental screening.

Develop a plan of care for individuals and families, modify treatment plans as indicated by client’s responses and conditions.

Provide referral/linkage when medical and environmental risks are identified to appropriate services.

Promote the program through community engagement and outreach events.

Develop and sustain community partnerships.

Deliver case management and nursing care according to public health protocols.

Perform physical assessments, take vital signs, fetal monitoring and perform basic lab screenings.

Provide breastfeeding education and counseling and refer clients to appropriate social and health services.

Maintain accurate records of patient care, services, and responses to treatment.

Other duties, as assigned.

Location: North Central Health District Office in Macon, GA with travel across several counties within the district. Employees in this role are eligible to receive mileage compensation for work‑related travel. Commuting to various counties such as Hancock, Washington, Baldwin, Wilkinson, Jones, Bibb, Peach, and occasional visits to Crawford, Houston, Jasper, Monroe, Putnam and Twiggs is required. Must be willing to travel to locations throughout the district as needed.

Qualifications
Associate's degree in nursing from an accredited college or university and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification. Must have a valid driver's license and reliable transportation.

Preferred Qualifications

Bilingual in Spanish and English

Bachelor’s degree in Nursing (BSN)

Experience providing interventions for high‑risk prenatal patients

Experience with high‑risk newborns and infants

Home visitation experience

At least one year of labor and delivery experience

At least one year of experience working in a prenatal care clinic

At least one year of experience working with mothers and infants

Employment Information
