Intake Coordinator- RN/ Social Work- Behavioral Health
Horizon health is seeking a clinical assessment coordinator to join our team at Ashtabula Regional Medical Center in Ashtabula, OH. This is for our 17 bed adult/ geriatric inpatient psych unit. The Clinical Assessment Coordinator, in conjunction with the Behavioral Health Program Director may coordinate the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients.
Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW
Facilitates groups, family and individual sessions and completing all necessary documentation
Schedules Unit assessments, meetings and appointments as appropriate
Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW
Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships
Participates as an active member of the interdisciplinary team
Maintains compliance with state and federal confidentiality laws as well as upholding our Horizon's confidentiality policies and procedures.
Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as requested
Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission and discharge data.
Assists in submission of reports and requests for documents from various regulatory agencies; re: PA Dept. of Health, CAP and Joint Commission
Prepares and disseminates monthly reports and completes required follow up on activities on behalf of the Director. Prepares and submits all operational tasks/documentation (census report, PIORS) as required by the corporate offices
Other duties as assigned

Qualifications
A Bachelor's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing or other related field is preferred; and/or
A Master's Degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing or other related field which includes a practicum in a health or human service agency is preferred
Minimum three years' of experience in a similar position and/or industry
