Registered Nurse Care Navigator - Concierge

Position Summary
The Concierge Care Navigator is a dedicated liaison for St. Luke’s concierge patients and other patient populations. This role ensures a seamless, personalized healthcare experience by triaging care needs, coordinating provider visits, and managing other care access points. The ideal candidate is a Registered Nurse with exceptional interpersonal, customer service, and communication skills, responsible for promoting teamwork across the healthcare team and performing duties consistent with St. Luke’s mission and values.

Education, Experience, & Licensing Requirements

Education: Bachelor’s degree in Science Nursing or related field.

Experience: Minimum of 3 years of clinical experience, preferably in ambulatory or care coordination settings.

Licensure: Current Registered Nurse license in Missouri. Certification in Case Management (CCM) or Patient Navigation preferred.
