Job Summary
Monitors and assesses the nutritional status of patients as it relates to disease state including patients on enteral nutrition. Counsels and instructs patients according to prescribed diet; must be able to adapt standard diets to individual physical, cultural and socioeconomic conditions and religious preferences. Assists with programs such as support groups, cooking demonstration and well as telephonic follow-up with patients as needed. Documents follow-up in EMR and works with multi-disciplinary team to support the needs of the population served. Keeps record of number of patients served and assists with meeting programmatic objectives.
This position provides nutrition counseling to individuals living with HIV who experience a wide variety of concerns, such as diabetes, hypertension, weight loss, and enteral nutrition.
Qualifications
Bachelor of Science in Nutrition from an accredited institution
Registered Dietitian License in the state of Georgia
