Description
CHJC’s Family Permanency Program (FPP) is funded through the New York State Office of Children and Family Services and serves families in Jefferson, Lewis, Oswego and St. Lawrence Counties. The target population is exclusively families who have finalized the adoption of their child(ren), families with legal guardianship of their child(ren), with or without a KinGap agreement, and families with formal or informal custody agreements.
Program Model
Promote permanency for post‑adoptive, post‑guardianship, and kinship families.
Provide assistance to families so children may be cared for in their own homes with their adoptive parent(s), legal guardian(s), or kinship caregiver(s); and
Strengthen post‑adoptive, post‑guardianship, and kinship families to avoid foster care or other out‑of‑home placements.
Essential Functions
Provides direct supervision of the Family Engagement Specialist (FES).
Monitors referrals to FPP and assigns to FES.
Meets with assigned staff for individual/group supervision, mentors, and professional development.
Monitors job performance and completes required performance evaluations.
Monitors staff schedules, time, attendance, and approves time off.
Monitors required on‑going professional development training for staff, completes staff development plans, and collaborates with HR to provide necessary training.
Provides on‑the‑job learning to all staff, documents observations, and provides feedback.
Completes all OCFS required reports.
Reviews work for timeliness, accuracy, and completeness, and signs off on case notes, intake documents, and discharge documents.
Assists in the hiring/interview process for new staff.
Assists assigned staff to mediate issues that arise among clients, families, and team members.
Completes assessments and creates treatment plans.
Provides clinical direction and consultation to FES to support implementation of treatment plan goals through non‑clinical interventions, skill‑building, and case management activities.
Translates clinical treatment plans into practical, developmentally appropriate strategies that FES can implement with families in home and community settings.
Guides FES in recognizing behavioral, emotional, and trauma‑related needs and responding appropriately within their non‑clinical role.
Supports FES in identifying barriers to engagement, progress toward treatment goals, and indicators for clinical reassessment or service modification.
May provide case coverage for short periods due to staffing needs.
Works collaboratively with CHJC Leadership to ensure treatment needs are met for enrolled youth.
Assists in recruitment and community education efforts.
Monitors and tracks peer support and family engagement activities.
Assists FES in creating, facilitating or identifying training for families.
Maintains a small clinical caseload of identified FPP youth to support program capacity and clinical continuity.
Provides monthly group supervision for PINS and WAC Caseworker.
Participates in the clinical on‑call rotation.
Requirements
Special Requirements:
Work Environment: Office environment with occasional travel to recruitment events, conferences and/or meetings and moderate travel within the community for off‑site/in‑home visits. May involve contact with service recipients who are agitated or in crisis. Some risk if working with recipients with mental illness. Requires occasional exposure to inclement weather against assignments.
Equipment:
Must maintain a working knowledge of related office equipment including personal computers, printers, audio‑visual equipment, telephone systems, copiers, fax machines, etc.
Physical Demands:
Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must perform fine motor skills, read, type, and sit for long periods. Reasonable accommodations may be made.
Position Type/Expected Hours Of Work:
Full‑time, traditional hours required, occasionally some non‑traditional hours. Must work on‑site and off‑site/in‑home visits.
Travel:
Required throughout the service area (Jefferson, Lewis, Oswego, and St. Lawrence Counties).
Required Education And Experience:
Current licensure in New York State Education Department as a Licensed Mental Health Professional (Licensed Psychologist, Licensed Clinical/Master Social Worker, Licensed Marriage and Family Therapist, Licensed Mental Health Counselor, or Licensed Creative Arts Therapist). Strong written and oral communication skills, excellent leadership, and time management skills.
Skills/Abilities/Knowledge:
Strong organizational and office skills. Excellent people skills, team player. Ability to work with privileged information ethically and unbiased. Ability to work with diverse populations and provide effective oral and written communication. Detail‑oriented, strong time‑management, self‑direction, tact, diplomacy, and professionalism.
Additional Eligibility Qualifications:
Must maintain privacy of confidential information. Successful completion of routine pre‑employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services and ongoing clearances and exclusion checks. Valid NYS driver’s license required. Corporate compliance, HIPAA privacy/security, workplace harassment and discrimination training, nonviolent crisis intervention, First Aid, and CPR will be required following hire.
