Active Home Health, Hospice & Personal Care is looking for a Full Time RN Admit Nurse. We are looking for someone who is caring, professional and self-motivated to join our team. Job Requirements
Initial Visit to assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.
Chart all clinical documentation in our EHR software
Create a patient plan of care & Coordinate services
Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
Provides medication education and management per physician order
Works hand-in-hand with the IDT and IDG to ensure patient goals are met and appropriate outcomes are achieved
Shares on-call duties
Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams
Job Qualifications
Registered nurse with current license to practice professional nursing in the state of Utah
Polite and compassionate
Excellent observation, problem solving, verbal and written communications
Maintains a current CPR certification
Must be a licensed driver with an automobile that is insured, registered and in good working order
Good communication and smartphone skills
Ability to pass a background check
