This assignment offers a weekly blended rate of $2,450, based on five 8‑hour day shifts. This role is perfect for a Case Manager who excels in discharge planning, interdisciplinary collaboration, patient advocacy, and coordination of complex care needs.

Key Responsibilities of the Case Management RN

Perform admission‑to‑discharge assessments, identifying patient needs and barriers

Develop individualized care plans with input from medical providers and support teams

Coordinate transitions to SNF, rehab, home health, outpatient therapy, and community resources

Facilitate utilization reviews and work collaboratively with insurance payors

Provide education to patients and families regarding follow‑up care and available support systems

Maintain timely, accurate documentation of case‑management activity

Advocate for patient safety, resource access, and continuity of care

Requirements of the Case Management RN

Active Florida RN license or compact eligibility

Current BLS certification

Minimum 1–2 years’ recent Case Management experience

Strong communication, assessment, and organizational skills
