CME Intake Specialist (Youth/Young Adults) - Behavioral Health (OhioRISE)

Be the First Connection for Youth and Families
Play a critical role in connecting youth and families to meaningful behavioral health services through OhioRISE. Coleman Health Services is seeking a skilled and compassionate professional to serve as a CME Intake Specialist, acting as the primary point of contact for referrals and ensuring timely, high-quality assessment and access to care. This position is located in our Akron, Ohio, office and serves Summit County.
What You'll Do
Serve as the primary point of contact for all OhioRISE youth referrals across assigned counties
Conduct CANS assessments
Coordinate intake, enrollment, and triaging of referrals to determine immediate needs
Support youth hospitalized due to behavioral health conditions by assisting with intake and transition planning
Communicate proactively with referral sources to gather collateral information and provide status updates
Ensure a smooth, supportive warm handoff to the assigned Care Coordinator following enrollment
Maintain timely, accurate documentation and tracking of referral and intake activity
Qualifications
High school diploma or equivalent
3+ years' experience providing community-based services to youth and their families
Valid driver's license
Ability to obtain NPI and Ohio Medicaid numbers (assistance available)
