We are looking for a compassionate and skilled healthcare professional to join our team and provide comprehensive family-centered care. In this role, you will be responsible for assessing, diagnosing, and managing a variety of health conditions across all age groups, from pediatric to geriatric patients. You will work collaboratively with our multidisciplinary team to promote health education and preventive medicine, ensuring that patients receive holistic care tailored to their needs.
Responsibilities
Conduct thorough patient assessments and health histories for a diverse patient population
Diagnose and treat acute and chronic illnesses, providing appropriate management plans
Perform routine physical examinations and preventive health screenings
Deliver health education to patients and families, fostering understanding of their conditions and lifestyle choices
Maintain accurate documentation of patient encounters and treatment plans
Collaborate with other healthcare professionals to coordinate patient care and referrals
Stay current with best practices in family medicine and participate in ongoing professional development
Qualifications
Valid Nurse Practitioner or Physician Assistant license in California
Master's degree in Nursing or Physician Assistant Studies
Experience in family medicine, primary care, or related fields preferred
Strong clinical assessment and problem-solving skills
Excellent interpersonal communication and patient engagement abilities
Ability to work effectively within a team-oriented environment
