We’re Hiring an RN – Hospice at Home with AdvisaCare
This isn’t rushed care. This is
presence , compassion, and honoring each moment.
Responsibilities
Provide one‑on‑one care in the comfort of home.
Support patients and families with dignity and heart.
Spend real time where it matters most.
Care without the chaos of hospital halls.
Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
Implement individualized plan of care and revisions as needed.
Compliance with AdvisaCare Hospice Policies and procedures, Medicare compliance.
Consult and educate patient/family on disease process, self‑care techniques, end‑of‑life care, dealing with ethical concerns.
Supervision of paraprofessionals providing services to patient.
Prepare clinical and progress notes, participate in IDG conferences.
Participate in hospice performance improvement program.
Maintain high level of customer satisfaction with clients, families and caregivers.
Communicate information using current process and technology within organization.
Schedule
Current on‑call hours: 5 pm – 8 am. Position requires on‑call responsibilities. Rotating schedule.
Requirements
Current Michigan License in good standing.
Minimum 1 year hospice experience.
Empathetic approach to patients and caregivers.
Participate in field visits daily, weekly office meetings as indicated.
Ability to adapt to patient’s status and needs on a full‑time on‑call basis.
