Overview
As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. You will continuously evaluate your performance, develop patient plans in collaboration with other professionals, and implement evidence‑based interventions that promote physical, psychosocial, emotional, and spiritual well‑being. Familiarity with the needs of patients and families who are facing death and dying is essential.
Primary Responsibilities
Makes the initial nursing evaluation in determining eligibility for hospice services within forty‑eight (48) hours of referral.
Identifies the patient/family’s physical, psychosocial, and environmental needs and reassesses at least every fifteen (15) days.
Initiates and coordinates the plan of care.
Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
Instructs and supervises the patient/family in self‑care techniques when appropriate.
Maintains accurate and relevant clinical notes regarding the patient’s condition.
Can function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Required Qualifications
Current and unrestricted RN licensure in the state of practice.
Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
State Specific Requirements – MS
1+ years of full‑time experience as an RN
Three years of full‑time clinical experience as an LPN may substitute for the one‑year RN experience.
Preferred Qualifications
1+ years of clinical experience.
Current CPR certification or ability to complete certification within 90 days of hire.
Compensation & Benefits
Salary for this role ranges from $60,200 to $107,400 annually based on full‑time employment. In addition to your salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility requirements). Salary and benefits are adjusted for local labor markets, education, work experience, and certifications.
