Social Services Coordinator

All positions require candidates to successfully pass a background check, Live Scan, and TB Test.

Responsibilities and Initiatives

Complete needs assessments and care plans promptly; coordinate referrals for mental health, housing, etc.

Facilitate discharge processes, including locating housing and completing discharge notes; conduct warm hand‑offs when appropriate.

Maintain up‑to‑date proprietary CRM and partner contacts; complete Coordinated Entry System (CES) documents, Homeless Management Information Systems (HMIS) records, and bed board applications.

Manage a minimum caseload of 25 guests, with flexibility to adjust based on facility needs.

Conduct home visits and rapid rehousing support as necessary to track post‑transition progress.

Adhere to administrative hours to complete required documentation and maintain a positive professional image in line with dress code.

Multi‑task and collaborate with external case managers to provide adequate services and support.

Triage and intervene in areas of guest behavioral concerns.

Ensure compliance with all funder and company standards, regulations, and best practices.

Support emergency service referrals (e.g., domestic violence, human trafficking, sexual assault) and provide warm handoff referrals.

Attend care coordination meetings and collaborate with nursing to conduct weekly case conferences.

Practice harm reduction and conduct searches for contraband or illegal substances, supporting interventions such as de‑escalation, crisis prevention, and motivational interviewing.

Engage with guests daily, building rapport and trust.

Follow all NHF policies and procedures consistently.

Qualifications

Bachelor’s Degree required; Master’s in social work preferred.

Minimum of three (3) years relevant experience in healthcare delivery, hospital education, social services, or a similar field.

Ability to present information effectively to directors, case managers, and discharge planners.

Excellent oral and written communication skills.

Positive, professional attitude with strong time‑management and organizational skills.

Computer proficiency in MS Word, Excel, Outlook, PowerPoint, and proficient typing skills.

Reliable transportation.

Bi‑lingual Spanish preferred.

TB test required; Fingerprint Live Scan/background check required.
