Job Summary
We are seeking a compassionate and dedicated Cardiology Nurse Practitioner (NP) to provide on‑site cardiovascular care to residents of skilled nursing and long‑term care facilities. The Cardiology NP will collaborate closely with cardiologists and interdisciplinary care teams.
Responsibilities
Provide on‑site cardiology consultations including initial and follow‑up evaluations
Assess and manage cardiovascular conditions such as hypertension, CAD, heart failure, arrhythmias, and valvular disease
Order, review, and interpret EKGs, echocardiograms, labs, and diagnostic studies
Collaborate with cardiologists and interdisciplinary teams to develop treatment plans
Manage and titrate cardiac medications per evidence‑based guidelines
Educate patients and caregivers on disease management and lifestyle modifications
Ensure accurate EMR documentation and regulatory compliance
Participate in quality improvement initiatives to reduce hospitalizations
Qualifications
Minimum 6 months experience in SNF/LTC or hospital/outpatient cardiology setting
Active and unrestricted Nurse Practitioner license
DEA license (Recommended)
BLS/CPR certification
Master’s degree in Nursing from an accredited NP program
Preferred cardiology experience (training available)
Strong communication, organizational, and time management skills
