Adult Partial Care Case Manager

Shore Haven provides treatment and support for adults who live with a severe and persistent mental illness as their primary diagnosis. Often times participants entering the program struggle with other co-occurring diagnoses, such as a particular substance use disorder. The Shore Haven program provides treatment six (6) hours each weekday, from 9am-3pm. Transportation arrangements can be made to accommodate program attendance.
Partial Care day treatment services exist for those people who find it difficult to maintain their lives in the community because of a behavioral health illness. It is utilized as a step-down for treatment following a hospitalization stemming from a mental health crisis.
We assist in connecting people to the community in a way that is valuable to them while improving overall behavioral and physical health. Our goal is to assist and support the participants in the program with reintegrating themselves back into society within their desired role in the community.

Responsibilities
Provide case management and maintain a caseload of clients diagnosed with SPMI
Assist with coordination of care for assigned clients with regard to medication management appointments, family collateral, linkage to community resources, and external referrals
Develop treatment plans and conduct treatment plan reviews
Facilitate skill building, supportive, and psychoeducational groups
Qualifications
Bachelor's Degree in a Human Services field
