Clinical Outreach Coordinator - Grace Medical Center - OP Behavioral Health
Department: FMF-OP BEHAVIORAL HE
Schedule: Full-time, Day shift, 8:30am-5:00pm
Responsibilities
Coordinate and provide intake screening for the Outpatient Mental Health Program, Partial Hospitalization Program (PHP), and Intensive Outpatient Program (IOP).
Determine appropriate services, schedule appointments and make referrals to other agencies as appropriate.
Gather clinical information from patients to link them to appropriate services.
Provide clinical guidance to referral sources.
Communicate clinical screening outcomes, symptoms, and behaviors to the treatment team.
Establish and nurture relationships with referral sources.
Market the programs by providing presentations to outside referral sources.
Coordinate outreach and interact with Lifebridge Health departments and public and private agencies.
Provide an excellent customer service experience to patients, referral sources, visitors, and co‑workers.
Requirements
Bachelor's Degree in Psychology or Social Work.
Minimum of 1 year of experience; 3+ years preferred – clinical experience working with SMI and chemically dependent patients.
Basic understanding of symptoms and pathology of mental illness.
