Responsibilities
Conduct comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing functions for residents.
Develop and implement individualized treatment plans; provide direct therapy and monitor progression toward goals.
Document evaluation findings, treatment plans, progress notes, and outcomes in the resident’s medical record; maintain confidentiality and regulatory compliance.
Collaborate with the interdisciplinary team to coordinate care plans, therapy approaches, and discharge planning.
Administer swallowing assessments when indicated; recommend texture modifications and compensatory strategies; provide patient and caregiver education.
Participate in care conferences and adjust goals based on progress and changing clinical needs.
Requirements
Master’s degree in Speech‑Language Pathology from an accredited program.
Active West Virginia state license to practice as a Speech‑Language Pathologist, or eligibility for licensure.
CCC‑SLP certification or eligibility preferred.
Ability to work in a travel assignment for 13 weeks and adapt to a new care environment.
Relevant clinical experience in rehabilitation or long-term care settings is preferred.
