Responsibilities

Advance client’s decision‑making, productivity and participation in a range of daily activities; perform other duties as assigned.

Facilitate the client’s inclusion in positive community experiences; transport clients to appointments and activities, ensuring safety and timeliness during travel.

Maintain current knowledge of client’s needs; sustain positive communications and relationships with supervisors, clients, landlords, employers, involved family members, the Regional Center, D.O.R., and others in the community.

Participate in the evaluation of the client through task analysis and other requested assessment devices.

Implement individual training and support programs under the direction of the case responsible person.

Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner.

Promote independence in daily activities while supporting clients with intellectual and physical disabilities; assist with meals, cooking, household maintenance, grooming, and dressing as assigned.

Address challenging behaviors proactively and respectfully, using methods outlined in plans and training.

Oversee client activities and environment during shifts to ensure health and safety; provide direct service as needed.

Qualifications

Experience: 18 years of age or older. Successful completion of pre‑employment drug screen and background check.

Schedule: A Direct Support Professional works as scheduled by the Regional Manager and/or designee. Overtime may be required; no holiday or overtime may be worked without prior approval of the supervisor.

License: Insured, registered vehicle in safe working condition; proof of current CA driver’s license.

Education: High School Diploma or Equivalent.

Management: Adjoin’s client case management platform requires employees to use a mobile smart device with reliable internet connectivity and data.

Compliance: Physical evaluations may be required, including TB test and/or lift and transfer test as applicable.

Skills: Aptitude for confidentiality, strong organizational and interpersonal skills.

Other: Ability to travel within the counties served (minimum 75% of the time); certification in First Aid and CPR (or successful completion within 45 days of hire).
