
Registered Nurse - Community Nursing Home

Overview
The Community Nursing Home (CNH) Care Coordinator Registered Nurse is a member of Geriatric & Extended Care (GEC) Services. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components.
Responsibilities
Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet complex health care needs.
Perform clinical nursing review of medical records of patients hospitalized in non-VA hospitals to determine point of stability for payment purposes and potential transfer to a VA facility.
Develop and coordinate seamless transitioning of patients throughout the inpatient and outpatient care process with a focus on discharge planning needs.
Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
Review and assist in patient referrals to CNH facilities to ensure continuity of qualified comprehensive care.
Participate as needed in discharge planning and community health services, including skilled nursing care, hospice, adult day health services, respite services, and activities required for successful transition to appropriate non-acute settings.
Maintain a current, valid state-issued driver's license; ability to operate a government-owned or leased vehicle as required.
Qualifications
Basic Requirements:
English Language Proficiency
Proficiency in basic written and spoken English (per 38 U.S.C. 7403(f)) for direct patient care positions
Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE
Options for licensure pathways include bridge programs, temporary appointments, and conversion to permanent after licensure completion
For graduates of foreign nursing schools: current, full, active, and unrestricted registration with a U.S. state, territory, or the District of Columbia; credential evaluation may be required for foreign education
Current, full, active, and unrestricted nursing registration in a U.S. state, territory, or the District of Columbia
GNT (Graduate Nurse Technician) exception allowances may apply during licensure pursuit
Grade determinations and education requirements are defined in VA qualification standards
Preferred experience: Geriatrics and Case Management
Physical requirements: ability to perform direct patient care duties, including potential long periods of walking/standing, lifting up to 35 pounds with Safe Patient Handling guidelines, adequate vision and hearing, and operation of a government vehicle
