Position Summary
The Post Acute Admissions Coordinator (LPN) provides daily onsite support at assigned hospitals to screen, evaluate, and coordinate appropriate post‑acute admissions on a full‑time basis. This role serves as a clinical liaison between hospital teams and the network’s post‑acute facilities, ensuring safe, timely, and clinically appropriate transitions of care.

Duties and Responsibilities
Including but not limited to:

Review and evaluate incoming referrals for sub‑acute rehabilitation and long‑term care placement via all platforms – EPIC, CarePort, Email/Community.

Conduct comprehensive clinical screening and evaluation of patients to determine appropriate level‑of‑care placement for long‑term care and sub‑acute admissions.

Complete and coordinate all admission requirements, including clinical documentation, financial applications, insurance verification, and required authorizations.

Prepare and submit complete admission packets, consulting with and obtaining approvals from the Director of Strategic Programming and the Lead Post‑Acute Coordinator as required.

Serve as the contact for post‑acute referral inquiries from GLIN community practices, hospitals, and care teams.

Maintain and update the referral and screening platform in alignment with organizational standards and operational workflows.

Complete admission screens in a timely, efficient, and clinically appropriate manner to support patient flow and throughput.

Review daily bed census reports and proactively coordinate placements to fill facility openings.

Apply sound clinical judgment and problem‑solving skills to determine appropriate placement and to elevate complex cases as needed.

Effectively communicate, assist, and collaborate with interdisciplinary teams, including hospital case management, social work, nursing, finance, and facility leadership.

Build and maintain strong working relationships with hospital partners, discharge planners, and facility staff to support seamless transitions of care.

Ensure compliance with organizational policies, payer requirements, and regulatory standards throughout the admissions process.

Participate in continuous improvement efforts to enhance admission efficiency, turnaround times, and patient experience.

Function as a mobile role, traveling between facilities as needed to support screening, admissions coordination, and relationship management.

Qualifications, Education, Training, and Experience

Licensed Practical Nurse (LPN) certification required.

1–3 years of experience in long‑term care, admissions, case management, or hospital discharge planning required.

5+ years of strong clinical assessment and critical‑thinking skills required.

1–2 years in a business office with SNF/Medicaid application experience preferred.

1–2 years of experience in admissions screening and long‑term care preferred.

1–2 years of proficiency in Microsoft Office Suite and Point Click Care, Epic preferred.

Valid NYS driver’s license required upon hire.

SCREEN certification preferred upon hire.

Pay range $25–$27 per hour. The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
