About the Position
Community Resource Specialist (Information & Assistance and State Health Insurance Assistance Program)

Responsibilities

Conduct client interviews, intake, and assessments to determine service needs and resource requirements; evaluate Medicaid eligibility (as they relate to Medicare) and educate beneficiaries on Medicare‑related issues.

Proactively support beneficiaries to resolve issues related to Medicare, Medicaid, private health insurance, high medical bills, access to care, and other health insurance administration concerns.

Serve as an agency representative with other service providers, address client needs, and make referrals to internal and external programs.

Advocate on behalf of clients as required, provide case plans, monitor and document plan status, and perform follow‑up on case services.

Coordinate the exchange of service information and provide recommendations to agencies; resolve conflicts and manage behaviors among clients, families, systems, and providers.

Monitor client status, conduct on‑site visits, evaluate safety and risk, determine crisis intervention needs, and adjust plans as client needs change.

Prepare, document, and maintain case data; develop reports that measure program outcomes and provide recommendations for improvement.

Provide training, community education, and outreach to groups and individuals; research community service needs and collaborate with stakeholders.

Deliver unbiased information on Medicare benefits and consult on options for older adults and adults with disabilities in the ADRC program.

Support the development and upkeep of record keeping, document management, and compliance with applicable laws and regulations.

Participate in special projects led by the Area Agency on Aging and other duties of a comparable nature.

Utilize digital accessibility techniques and modern office technology to support client interactions and documentation.

Education and Experience Requirements

Bachelor’s degree in gerontology, social work, or a related human services field and one to three years of related human services experience, or an equivalent combination of education and experience sufficient to perform the essential duties of the job.

Compensation and Benefits
Hiring salary range: $4,535.46–$5,442.26 monthly or $2,267.73–$2,721.13 semi‑monthly, depending on qualifications.

Licensing / Certifications

Certified Information & Referral Specialist for Aging/Disabilities (CIRS‑A/D) certification.

State Health Insurance Assistance Program and Senior Medicare Patrol certification within 60 days of hire (on‑job training provided).

Valid Colorado Driver’s License and access to a vehicle or an equivalent alternative transportation.
