We are currently seeking our next licensed Vocational Nurse to join our team!

We have a fantastic opportunity for a Licensed Vocational Nurse to provide person‑centered, individualized, and clinically competent care with support from the management team to all residents, both temporary and permanent, ensuring services meet residents’ needs per federal, state, and local regulations.

This role pays $32.00 - $37.66 / hr DOE

What You'll Do

Understand the importance of resident rights in healthcare as well as the community as a whole, and advocate for operations, processes, and decisions that are in residents’ best interests.

Delegate and assign appropriate tasks to team members.

Work closely with team members by ensuring accountability, communication, and effective work, including following through on Service Plans, skin checks, completion of care, and prevention of falls.

Provide medication and treatment administration for an assigned group of residents.

Manage medication administration by maintaining standards of practice, understanding universal precautions, setting up and passing medications, and observing residents for drug effects.

Identify, gather information and report changes in resident conditions.

Document findings, notify physician, and create a resident‑specific and person‑centered Service Plan to address changes and needs.

Receive, transcribe, and review physician orders for accuracy including reconciliation of all medications, including controlled substances.

Write and develop all Service Plans so that they identify resident needs, approaches, and goals to be met.

Read and follow individualized care and Activities of Daily Living (ADL) plans and promote team focus on maintaining the resident’s highest level of functioning.

Support in the admission of new residents, including assisting with resident needs, educating caregivers, and ensuring completion of admission assessment and notes.

Promote resident independence to the greatest degree possible.

Enhance and maintain a physical, social, and psychological environment conducive to the best interest and welfare of the residents.

Review and maintain 24‑hour reports and other pertinent nursing documentation, identify areas requiring action, and follow up as needed.

At the end of shift, provide updated 24‑hour report.

Attend and participate in continuing education programs and in‑services.

Ensure proper documentation by entering resident information into electronic medical records in a timely and accurate manner.

Be constantly aware of safety and follow up on identified risks.

Respond to emergency calls on campus per protocol.

Initiate and complete incident reports for any accidents, medication errors, emergency situations, etc.

Assist team members with direct care as needed.

Supervise, assist, and implement infection control measures and prevention to ensure compliance.

Perform other duties as assigned.

What You’ll Need

Minimum of one year’s professional nursing experience, especially in long‑term care.

Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license from an accredited nursing program, school, or university.

Current CPR certification is required.

Demonstrated understanding of the functions, organization, policy, regulations, and procedures of a long‑term care/skilled nursing facility.

Competent in use of electronic health records and other clinical software.
