Job Summary
The Provider, Direct Care, will implement and maintain a Community Integration program for all assigned consumers to achieve their individual goals and provide intensive support services to develop the abilities necessary to maximize independent functioning in a home and community environment.

Essential Functions

Direct Care Services Program

Meet with potential new clients to interview as a potential provider.

Complete all client program paperwork, including Individual Support Program (ISP) tracking, monthly timesheets, and incident reports.

Provide services specified in the Individual’s Plan and assist clients with working toward their goals, documenting progress.

Travel is required, conducted in a company vehicle.

Client/Program Tracking and Reporting

Complete all necessary client-specific documentation on a monthly basis.

Maintain complete and accurate records and statistical data as required for all clients and programs.

Relationship Management

Participate in required training sessions.

Deal effectively and calmly with non‑routine situations following accepted guidelines and perform any additional duties as required by the Program Coordinator.

Perform direct service tasks outlined in the plan of care, such as personal care, emotional support, housekeeping, and home management.

Qualifications

Education : High School Diploma or equivalent is preferred.

Experience : Experience in a related field or equivalent combination of training and experience in a social service, counseling, or rehabilitation setting, providing direct services to persons with physical, emotional and/or developmental disabilities.

Other

Ability to write clear, concise, and accurate reports and plans, submit them in a timely manner, and possess excellent time‑management skills.

Excellent written and verbal communication skills and the ability to interact with various types of internal and external customers.

Working knowledge of community resources and ability to motivate consumers toward individual success.

Professional appearance and conduct that enhances the firm's reputation.

Empathy toward disabled persons, their parents, and guardians.

Independent and cooperative work capability with effective planning.

Flexibility within the framework and ability to meet established deadlines.

Driving capability for company business.

Benefits

Pay: $17/hour

Daily Pay eligibility through DailyPay.

Part‑time – 12 hours per week; client specific.

Paid Sick and Safe Time (PSST) under the Healthy Families and Workplaces Act.

Retirement plan contributions (pre‑tax or post‑tax) for employees 18+.

Eligibility for company vehicle assistance and mileage reimbursement.

Reaching, twisting, bending, kneeling/crawling, squatting, climbing, hearing, vision, hands/fingers, walking, standing, sitting, speaking, and occasional driving are continuous or frequent requirements as described in the job specification.
