
RN - Continuous Care RN - Hospice - Days

Job Overview
Horizon Home Care and Hospice seeks experienced nurses to join our hospice team as Continuous Care Registered Nurses. In this role you will be the primary point of contact for patients and their families, providing comfort, symptom management and support at the end‑of‑life journey.
Responsibilities
Assess patient conditions accurately and completely during scheduled visits (physiological, psychosocial, and/or spiritual).
Provide interventions for symptom management during in‑home visits and evaluate patient response prior to the end of your shift.
Identify and provide patient and family education and support.
Complete all required patient documentation (visits) in an accurate, legible manner on the same day care was provided.
Communicate and provide reports to physicians, managers, and interdisciplinary team members of any changes in patient condition or the plan of care in a timely and accurate manner.

Qualifications
Degree from an Accredited School of Nursing (ASN or BSN)
Current Wisconsin RN license
At least 1 year of experience as an RN in acute care, long‑term care (geriatric), home care, or hospice setting
Valid Wisconsin driver’s license and reliable insured vehicle
Part‑time position: 36 hours per week. Day shift (8:00 am – 8:00 pm) plus every other weekend and holiday rotation.
Salary: Exempt.
