About Mass General Brigham Home Care
Mass General Brigham Home Care provides post-acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery, or injury and manage exacerbations of chronic disease. Our clinicians are highly skilled professionals who function independently while collaborating as part of a strong interdisciplinary team to deliver the highest quality patient-centered care.
Responsibilities
Deliver 1:1 physical therapy care in patients’ homes functioning as a Case Manager
Evaluate patients and develop personalized treatment plans
Focus on mobility, strength, fall prevention, and functional independence
Assess home environments to promote safety and independence
Educate patients and caregivers on recovery strategies and long-term mobility
Collaborate with nurses, therapists, physicians, and social workers to support recovery
You won’t just treat symptoms — you’ll help patients regain independence and confidence in their own environment.
Geographic Coverage
Primary Coverage Area: Most of your time will be spent in Somerville and Medford, with additional support in Arlington, Everett, Malden, and Melrose. We will secure city parking passes for new team members to facilitate commute.
Schedule & Workload
Part-time (32 hours)
Average of 5–6 visits per day
Dedicated time for documentation, chart review, visit preparation, and care coordination
Set your own schedule by managing your own appointments, while providing high-quality, one-on-one patient care.

Job Summary
Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.
Key Activities
Develop a plan of care that is patient and family focused and institutes standardized interventions for goal-oriented outcomes
Daily documentation is accurate and timely
Application of standard treatment procedures
Teaching is provided to patients and caregivers
Qualifications
Qualifications and Experience
One year of experience in acute care, rehab or homecare as a Physical Therapist
Registered with the Board of Registration in Massachusetts
Excellent verbal, written and electronic communications skills
Computer proficiency and ability to learn new software
Ability to prioritize work and adapt to changing situations
Travel required within Mass General Brigham Healthcare at Home’s geographic area
