Job Summary and Qualifications
The Transplant Coordinator I is a professional registered nurse who performs a variety of clinical services and patient care activities in collaboration with a multidisciplinary healthcare team. The coordinator works with patients and their families from transplant evaluation through post‑transplant care, ensuring a smooth flow of the entire process with compassion and tact.

Essential Job Functions

Assess patient needs, including psychological, physical, environmental, and functional assessments.

Educate patients and families on disease processes and the transplant journey, serving as a resource for education across staff, hospital, and community.

Communicate effectively with all transplant team members, including hospital staff and referring physicians/staff.

Collect, enter, and manage data in the patient medical record to meet regulatory agency requirements.

Document assessments, interventions, and follow‑up accurately and in a timely manner.

Coordinate care through all phases of transplantation, including pre‑transplant assessments, educational processes, patient selection, monitoring during work‑up and listing, preparation for surgery, and postoperative follow‑up.

Lead educational initiatives for patients and families, and serve as the primary resource on transplantation for all staff.

Participate in clinical rounding to monitor quality and identify adverse changes.

Maintain communication with the multidisciplinary team and manage transitions from inpatient to outpatient care.

Monitor and follow diagnostic studies post‑transplant, evaluate health status per clinical protocol, and coordinate any patient issues with the transplant team.

What Qualifications You Will Need

Graduate of an accredited registered nursing program (BSN preferred).

Minimum one year of nursing experience.

Minimum License, Certificates, or Registrations Required

Valid Texas RN license.

BCLS 2.
