Ennoble Care is a mobile primary care, palliative care, and hospice service provider serving patients across multiple states. Our clinicians visit patients’ homes to provide continuum of care for chronic conditions and limited mobility. We offer remote patient monitoring, behavioral health management, and chronic care management programs.

Position Overview
The Care Coordinator, Embedded is a full‑time, in‑person member of the clinical care team who supports Ennoble Care providers within long‑term care facilities in the Reston, Virginia area, focusing on the Front Royal vicinity.

Responsibilities

Coordinate care for long‑term care residents using Ennoble Care’s EMR.

Maintain regular, proactive on‑site engagement with residents to support care plans, follow‑ups, and care transitions.

Collaborate closely with Ennoble Care providers within the long‑term care setting.

Serve as a liaison between providers, facility staff, residents, and families.

Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.

Accurately document patient interactions and care coordination activities.

Follow established workflows, documentation standards, and escalation protocols.

Contribute positively to team collaboration and strong facility partnerships.

Embedded Duties
This role is not a remote or call‑center position. The Coordinator is physically present in one or more partnered long‑term care facilities, building consistent relationships with residents, families, providers, and staff.

Requirements

Legally authorized to work in the United States.

Active, unrestricted clinical certification or licensure (e.g., Medical Assistant, Certified Medical Assistant, Licensed Practical Nurse, Registered Nurse).

Minimum 1 year of patient‑facing experience in a long‑term care, skilled nursing, or similar clinical setting.

Excellent communication skills via Microsoft Teams, email, phone, and in person.

Strong organizational and time‑management skills; proactive, initiative‑driven approach.

Working knowledge of healthcare terminology and care‑coordination principles.

Proficiency with Microsoft 365 (Outlook, Word, Excel).

Availability to work full‑time, Monday through Friday 8:00 a.m. to 4:30 p.m. EST (facility hours may vary) in an on‑site role based in the Reston, Virginia area.

No on‑call; daytime hours only.

Seven paid company holidays annually.

Valid driver’s license and reliable transportation to fulfill duties; may require travel between facilities.

Current auto insurance and ability to provide proof.

Must pass a background check and drug screen.
