Care Coordinator, Embedded (Reston, VA - Springfield)
Location: Reston, VA
Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across the United States. Our clinicians provide care in patients’ homes, offering a continuum of care for chronic conditions and limited mobility.
We offer programs including remote patient monitoring, behavioral health management, and chronic care management to ensure the highest quality of care.
Key Responsibilities
Coordinate care for long‑term care residents using Ennoble Care’s EMR.
Maintain regular, proactive, on‑site engagement with residents to support care plans, follow‑ups, and care transitions.
Collaborate closely with Ennoble Care providers within the long‑term care setting.
Serve as a key liaison between providers, facility staff, residents, and families.
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.
Accurately document patient interactions and care‑coordination activities.
Follow established workflows, documentation standards, and escalation protocols.
Contribute positively to team collaboration and strong facility partnerships.
What 'Embedded' Means
This is not a remote or call‑center role. As an Embedded Care Coordinator, you are physically on‑site in one or more partnered long‑term care facilities, working alongside providers and facility staff. You build consistent relationships with residents and families and become a trusted, familiar presence within the facility.
Requirements
Legally authorized to work in the United States.
Active, unrestricted clinical certification or licensure (Medical Assistant, Certified Medical Assistant, Licensed Practical Nurse, or Registered Nurse).
Minimum 1 year of patient‑facing experience in a long‑term care, skilled nursing, or similar clinical setting.
Comfortable communicating proactively and professionally via Microsoft Teams, e‑mail, phone, and in person.
Ability to cultivate positive, approachable relationships with residents, families, providers, and facility staff.
Well‑developed organizational and time‑management skills, optimistic approach, and initiative‑driven mindset.
Strong working knowledge of healthcare terminology and care‑coordination principles.
Proficiency with Microsoft 365 (Outlook, Word, Excel).
Ability to work full‑time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, on‑site in the Reston, Virginia area (Springfield vicinity).
No on‑call and daytime hours only.
Seven company‑paid holidays annually.
Valid driver’s license, reliable transportation, current auto insurance; travel between facilities may be required.
Ability to pass a background check and drug screen.
