Care Coordinator, Embedded (Reston, VA - Woodburn)

Ennoble Care is a mobile primary care, palliative care, and hospice service provider that visits patient homes. We seek a full‑time Care Coordinator to support our clinical team in long‑term care facilities, ensuring continuity of care and building strong relationships with residents and families.
Key Responsibilities
Coordinate care for long‑term care residents using Ennoble Care’s EMR.
Maintain regular, proactive on‑site engagement with residents to support care plans, follow‑ups, and care transitions.
Collaborate closely with Ennoble Care providers and long‑term care staff.
Serve as a key liaison between providers, facility staff, residents, and families.
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.
Accurately document patient interactions and care coordination activities.
Follow established workflows, documentation standards, and escalation protocols.
Contribute positively to team collaboration and strong facility partnerships.
Requirements
Legally authorized to work in the United States.
Active, unrestricted clinical certification or licensure (e.g., Medical Assistant, Certified Medical Assistant, Licensed Practical Nurse, Registered Nurse).
Minimum 1 year of patient‑facing experience in long‑term care, skilled nursing, or a similar clinical setting.
Comfortable communicating proactively via Microsoft Teams, email, phone, and in person.
Strong organizational and time‑management skills; optimistic, initiative‑driven mindset.
Working knowledge of healthcare terminology and care coordination principles.
Proficiency with Microsoft 365 (Outlook, Word, Excel).
Ability to work full‑time Monday‑Friday 8:00 a.m. to 4:30 p.m. EST on‑site in the Reston, Virginia area with a primary focus on Woodburn; no on‑call required.
Valid, unencumbered driver’s license and reliable transportation; able to provide proof of auto insurance.
Ability to pass background check and drug screen.
Will be required to do occasional travel between facilities.
No need for immigration sponsorship before offer.
