Care Coordinator, Embedded (Reston, VA - Warrenton)
Location: Reston, VA
Join Ennoble Care, a mobile primary, palliative, and hospice care provider delivering comprehensive services throughout the United States. As a Care Coordinator, Embedded, you will provide on-site support within partnered long‑term care facilities in the Reston, Virginia area with a focus on the Warrenton vicinity.
Key Responsibilities
Coordinate care for long‑term care residents using Ennoble Care’s EMR.
Maintain regular, proactive, on‑site engagement with residents to support care plans, follow‑ups, and care transitions.
Collaborate closely with Ennoble Care providers within the long‑term care setting.
Serve as a key liaison between providers, facility staff, residents, and families.
Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.
Accurately document patient interactions and care coordination activities.
Follow established workflows, documentation standards, and escalation protocols.
Contribute positively to team collaboration and strong facility partnerships.
Requirements
Legally authorized to work in the United States.
Active, unrestricted clinical certification or licensure, such as Medical Assistant (MA)/Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), or Registered Nurse (RN).
Minimum 1 year of patient‑facing experience in a long‑term care, skilled nursing, or similar clinical setting.
Comfortable communicating proactively via Microsoft Teams, email, phone, and in person.
Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff.
Well‑developed organizational and time‑management skills.
Strong working knowledge of healthcare terminology and care coordination principles.
Proficiency with Microsoft 365, including Outlook, Word, and Excel.
Ability to work full‑time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST (or as needed to accommodate facility hours).
No on‑call duty.
Daytime hours only.
Seven (7) company‑paid holidays annually.
Transportation: valid, unencumbered driver’s license; consistent transportation; current auto insurance with proof available.
Ability to pass a background check and drug screen.
