Job Summary
The University of Colorado College of Nursing is seeking a Bilingual Primary Care Referral Coordinator for a full‑time (1.0 FTE) position within the Office of Clinical and Community Affairs at the Sheridan Health Services clinics in the Fort Logan neighborhood of Denver. The Bilingual Primary Care Referral Coordinator performs referral and administrative duties to support comprehensive, high‑quality patient care, representing Sheridan Health Services professionally and positively.
Clinic Description
Sheridan Health Services is a multi‑site, nurse‑managed federally qualified community health center funded through the Health Resources and Services Administration Bureau of Primary Health Care. It serves community health needs for residents of Arapahoe, Denver, and Jefferson counties. The Family Health Clinic serves adults and children, 65% Latinx, 42% best served in another language, with Spanish predominant. Services include primary care, prenatal, postpartum, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance use treatment. Clinic supports patient‑centered model and community outreach.

Supervision
Direct supervision by the APN Clinical Lead.
Responsibilities
Referral Processing (75%)
Process routine, urgent, and emergent referrals for Adult Primary Care Service Line.
Verify patient insurance eligibility; communicate with nurse practitioners and the healthcare team as needed.
Obtain prior authorization or pre‑certification when required.
Assist with scheduling appointments consistent with patients' insurance plans.
Prepare patient paperwork for provider review and finalization.
Track referrals until completion per clinic policy.
Connect with specialty care clinics, community resources, and services.
Communicate diagnostic and clinical information to complete referrals thoroughly.
Uphold HIPAA policies and procedures to protect patient health information.
Attend community events to assist in care coordination or perform community outreach.
Serve as point of contact for community partners to manage incoming referrals and schedule new or existing patients with more than one service line.
Administrative (25%)
Monitor incoming faxes daily and upload progress notes, scheduling notices, imaging results, and specialty referral communication to the EMR.
Assist in managing transitions of care from the Emergency Department and hospitalizations to primary care.
Assist in scheduling clinic visits as needed.
Prepare or manage Medical Records Requests as requested.

Qualifications
Minimum Qualifications – Experience Only
Three (3) years general clerical experience, including one (1) year caring coordination in a clinical or insurance referral unit.
OR Education and Experience
A combination of related education/experience equal to three (3) years.
College/university coursework may substitute on a year‑for‑year basis.
Applicants must meet minimum qualifications at the time of hire.
Applicants must be legally authorized to work in the United States without requiring sponsorship. This position does not provide visa sponsorship.
Must obtain current BLS/CPR certification by hire date.
Must speak Spanish and English daily to communicate clinical terminology.
Preferred Qualifications
Experience in Federally Qualified Health Center (FQHC).
Experience as Medical Assistant, Referral Coordinator, or Patient Navigator.
Knowledge, Skills, and Abilities
Effective written and oral communication.
Establish and maintain effective working relationships across the organization.
Outstanding customer service skills.
Attention to detail.
Commitment to person‑centered care.
Knowledge of basic medical terminology and experience with EHR systems.
Problem solving and decision‑making skills.
