Job Summary and Responsibilities
The Rehab Liaison Therapist is responsible for maintaining current referral sources and for developing new clients in order to create a significant referral base for maintaining a set inpatient population. The Rehab Liaison Therapist, in consultation with St. Mary Medical Center physicians, is also responsible for the completion of appropriate pre-screening documentation.
Promotes service excellence that will maintain/improve customer satisfaction.
Creates a marketing strategy in collaboration with St. Mary Medical Center Public Relations and Unit Director.
Including program presentations to new networks.
Assures that financial screening and authorizations are complete, current, and accurate prior to patient's admission to Acute Rehab.
Accurately completes pre-admission screens on all patients transferred to the Acute Rehab Unit.
Implements marketing strategy.
Coordinates with the lead nurse to make appropriate room assignments for new admissions.
Assures that all necessary department files regarding admissions and discharges are accurately maintained.
Provide assistance to the team members having difficulty dealing with provider C.M. Collaborates with the CSW and Therapy team as to the provider's C.M./contact person name and telephone number, enter all other necessary information on the Discharge Instruction Guide and filed on the DME binder.
Resolves conflict in a manner that confronts the issues through effective listening, assertive skills, patience and understanding.
Maintains effective and open communication using basic verbal and nonverbal skills in a professional way to the patients and their families as well as to all St. Mary Medical Center customers.
Maintains effective and open communication and support with Nursing Services throughout the Medical Center.
Job Requirements
Rehabilitation professional with a minimum of 5 years Case Management and clinical Rehabilitation experience.
Bachelors Degree
Occupational Therapist: CA (OT:CA), upon hire, Required OR
Physical Therapist: CA (PT:CA), upon hire, Required OR
Speech Language Pathologist: CA (SLP:CA), upon hire, Required
