FOX Rehabilitation is seeking a compassionate and motivated Physical Therapist to join our clinician-owned, geriatric-focused practice serving patients in the Greater Mount Laurel, NJ area. In this community-based role, you will deliver evidence-based, one-on-one physical therapy services to older adults across a variety of settings, including private homes, senior living communities, and other residential environments. You will perform comprehensive evaluations, develop and implement individualized treatment plans, and focus on improving functional mobility, strength, balance, gait, and overall quality of life. Responsibilities include conducting detailed geriatric and orthopedic assessments, creating and progressing therapeutic exercise programs, providing balance and fall-prevention interventions, and training patients in safe functional mobility, transfers, and use of assistive devices. You will document care accurately and efficiently in an electronic medical record, track functional outcomes, and adjust plans of care based on clinical findings and patient goals. As a FOX Rehabilitation clinician, you will collaborate closely with an interdisciplinary team, including physicians, nurses, occupational therapists, speech-language pathologists, and family members, to ensure comprehensive, patient-centered care. You will educate patients and caregivers on home exercise programs, safety strategies, and lifestyle modifications that support long-term independence.
