Registered Nurse – RN Discharge Lounge

Deliver patient-centered care during admissions, discharges, and transitions for acute care patients, ensuring timely communication with medical teams and thorough patient education.

Job Summary
Responsible for seamless admissions and discharges, accurate documentation, and patient education while advocating for patients and families.

Responsibilities

Obtains a nursing assessment history and ensures proper inpatient care unit admission, taking appropriate action when needed, including communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.

Completes and facilitates required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.

Teaches patients/families/caregivers about the patient’s medical condition, status, treatment plan, medications, possible side effects, and follow-up measures, translating complex medical terminology to ensure complete understanding.

Completes extra discharge documentation, education, and discharge care for special patient populations to ensure compliance with core measures for discharge.

Ensures patients/families/caregivers receive copies of discharge paperwork and any other required documentation, such as referrals for ongoing care and educational materials.

Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.

Demonstrates time-conscious awareness and prioritization, distinguishing between low- and high-priority activities.

Qualifications

Basic Cardiac Life Support required within 30 days of employment start date.

Advanced Cardiac Life Support (ACLS).

Registered Nurse (RN) license.

Master’s Degree or Bachelor’s Degree in Nursing.

Minimum 1 year of experience.
