Job Description
Swope Health is seeking a compassionate and mission-driven Community Health Worker to be part of a team of caregivers addressing social determinants of health and focusing on reducing financial barriers that impact access to care. The role involves assisting individuals and families in enrolling in public benefits such as Medicaid and the Affordable Care Act (ACA) marketplace, connecting them to critical resources, and providing financial literacy education.
Primary Responsibilities
Identify individuals and families experiencing financial and social barriers that limit access to healthcare, medication, housing, food, and transportation.
Provide culturally responsive support to help patients overcome obstacles to care.
Assist individuals and families with understanding eligibility requirements and completing applications for Medicaid and ACA Marketplace health insurance.
Provide education on coverage options, subsidies, renewal processes, and rights/responsibilities under available programs.
Follow up to ensure successful enrollment, renewal, and linkage to ongoing care.
Link patients to community resources addressing financial, housing, food, and transportation needs.
Guide individuals through applications for programs such as SNAP, WIC, housing assistance, utility assistance, and other support services.
Deliver one-on-one or group education on budgeting, healthcare costs, and resource management.
Act as a patient advocate in navigating healthcare systems, insurance, and community services.
Qualifications
High school diploma or GED equivalent.
Bilingual (English/Spanish).
Certification or formal training as a Community Health Worker highly preferred.
Certified Applications Counselor (CAC) certification highly preferred.
Patience and ability to establish rapport with this population.
Flexible schedule and transportation.
Knowledgeable about diverse clientele.
Ability to write clearly and distinctly.
