Practice Overview
Location: Lancaster, California Setting: Community Health Center (Outpatient Internal Medicine)⚕️ Focus: Adult primary care, preventive medicine, and chronic disease management Collaborative, team-based clinical environment Your Role You will provide comprehensive internal medicine care, including:✔️ Diagnosis and management of acute and chronic adult conditions✔️ Preventive care and health maintenance visits✔️ Chronic disease management (diabetes, hypertension, cardiovascular risk, etc.)✔️ Medication management and care coordination✔️ Patient education and long-term care planning Patient Population ⚕️ Adult and geriatric patient population Underserved community with diverse healthcare needs High focus on preventive care and chronic disease control Requirements ✔️ MD or DO in Internal Medicine✔️ Board Certified or Board Eligible in Internal Medicine✔️ Active or eligible California medical license✔️ Strong interest in community-based outpatient care✔️ Commitment to serving underserved populations Compensation & Benefits
