Summary of Position
The Program Manager is responsible for managing various department programs providing patient support and linkage to care for a variety of priority populations. Programs include but are not limited to HIV/AIDS, Viral Hepatitis (HBV/HCV), Pre‑Exposure Prophylaxis (PrEP) / Post‑Exposure Prophylaxis (PEP), Cancer Prevention, Reproductive Life Planning, General Community Health and Health Promotion. The Program Manager will implement and oversee assigned programs, track measurable outcomes, monitor program sustainability, and report to partner agencies. The role must develop and implement strategies to increase screenings, linkage to care and utilization of care and treatment for patients served. In addition to managerial duties, the Program Manager will provide individualized case management and/or care coordination, including patient navigation, education, resources, referrals and advocacy for patients and their families.

Communicate effectively with a wide range of patients, operations staff, providers and other departments in a friendly, professional manner incorporating a team‑based approach to healthcare

Knowledge of medical office protocols/procedures

Plan, prioritize and complete delegated tasks with attention to detail and efficient time management

Work well under pressure and with various time constraints

Learn and retain information regarding patient care procedures

Proficiency in computer operations, including MS Word, Excel, Outlook, PowerPoint

Work successfully with indirect supervision and autonomy

Work with culturally diverse people

Typical work hours are from 8:00 am to 5:00 pm; attendance at evening and/or weekend meetings may be required

Must have valid driver’s license and personal transportation to travel between office/county locations

Neat, well‑groomed and professional appearance

Education
Bachelor’s degree in Social Work, Public Health or a related discipline; Master’s degree preferred.

Experience
Minimum 3‑year experience working within a clinical setting (public/community health) and/or with priority communities/patient populations. Supervisory experience preferred.

Additional Skills
Bilingual.

Professional Licensure
N/A.

Certification
Basic Life Support.

Key Responsibilities

Demonstrate and apply knowledge of medical conditions, terminology, transmission, risk reduction, disease referral resources and accessible treatment programs

Update/maintain accurate and secure patient medical records

Perform medical case management and/or care coordination duties: intake/assessments on new and established patients

Maintain up‑to‑date progress notes completed timely, according to department policy

Develop comprehensive plans of care (working with patients and providers) to include goal setting that achieves and maintains optimal patient health while advocating in the best interest of the patient & organization

Coordinate/facilitate referrals to link patients to care/resources

Perform community outreach to promote programs, establish partnerships, attend community events, coordinate community meetings and provide educational presentations

Provide programmatic oversight and leadership to assigned team, including responsibilities of assigned program(s), communication with assigned team and identification/solving of process or personnel challenges

Complete and submit required programmatic reports to funders in a timely manner according to contracts, and monitor programmatic expenditures and requirements to ensure contractual compliance

Actively participate in QI activities to evaluate program activities to improve quality of care and program efficiencies, monitoring progress toward goals

Participate in staff/committee/agency meetings as required; lead meetings/huddles as requested

Assist with developing programming and new initiatives based on team and patient needs (e.g., support groups, educational sessions, new partnerships)

Regularly update Supervisor with changes or challenges to program(s) and perform other clinical or administrative functions per the Dept. Director
