Re-entry Program Coordinator Non-Exempt Position
Responsible To
Program Director
Major Duties
Coordinate with the RCS Program Administrator from the Division of Behavioral Health and Recovery to obtain referral packet and designation letters for assigned participants.
Coordinate with Department of Corrections to attend pre-release meetings and develop a transition plan for coordination of care if participant is still in custody.
Work with the participants in creating and maintaining a budget, accessing support funding, and ensuring requirements of the RCS program are being followed.
Maintain complete and up-to-date clinical files that document all services and contacts provided consistent with the requirements of WAC 246-341.
Review Washington Administrative Codes (WACs) and Revised Codes (RCWs) and Federal regulations pertaining to mental health treatment to ensure compliance with these regulations in all therapeutic services.
Support and implement agency policies and procedures.
Provide referrals to mental health, substance use, or other treatment services as needed.
Facilitate monthly RCSP meeting with community partners to discuss participant status.
Maintain accurate financial records for purchases made using supportive funding.
Maintain monthly service summary report.
Represent the Agency in the community at various meetings with the approval of the supervisor.
Other duties as assigned.
Knowledge and Skills
Bachelor's degree in human sciences or related field preferred.
High school diploma or equivalent with relevant college coursework required.
One year experience working with adults or youth suffering from co-occurring mental illness and substance use dependency required.
Familiarity with applicable Washington State regulations related to the provision of mental health services (RCW 71.05) and substance use dependency services (WAC 246-341).
Guidelines
This position requires that the incumbent obtain and maintain Washington State credentials through the Department of Health.
Complexity
The incumbent provides agency services to participants who differ widely in age and socioeconomic status and who may possess a variety of chronic and serious social, behavioral and psychological problems. Strong communication skills, sound judgment, and creativity are required.
Personal Contacts
Contacts are with the members of the treatment team, representatives of various community agencies specializing in the treatment of chemical abuse and mental illness, and related community representatives including hospital personnel, lawyers, probation officers, and other court officials. All personal contacts are carried out in accordance with federal and state laws dealing with the confidentiality of patient records.
Physical Demands
While performing the essential functions of the job, the employee is regularly required to sit, speak clearly, hear and use hands and fingers to manipulate writing utensils and keys on a keyboard.
Working Environment
Working hours are spent indoors in offices or other meeting rooms. Supervision of outdoor activities, visits to community agencies, and participation in staff retreats and staff development activities may be required.
