Community Relations Manager

who wants to be the reason more patients find the care they deserve.
This isn’t a desk job. You’ll be in the field — walking into hospitals, sitting down with discharge planners, showing up at community health fairs, and building the kind of trust that turns a first conversation into a lasting partnership. When a family needs home care, you’re the person who makes sure they think of People Care.
Your day looks like this:

Building real relationships with MLTC plans, hospitals, community organizations, and referral partners across your territory
Representing People Care at community events, health fairs, and partner meetings — you’re our face in the neighborhood
Creating outreach strategies that actually fit the communities you serve — their languages, their culture, their needs
Working side by side with our Intake, Coordination, and Clinical teams to make sure every referral becomes real care for a real person
Bringing back what you hear in the field — the feedback, the trends, the opportunities — so we get better
You might be the right person if:

You have

3+ years in

community outreach, partnership development, or healthcare liaison work — and you loved the relationship-building part most
You know the

home care world

— the patients, the payers, and how referrals actually work
You’re the kind of person people remember after one meeting
You can present to a room, connect with a family, and follow up on a handshake — all in the same day
Bilingual English/Spanish or English/Haitian Creole? Even better
