Title:

RN Field Coordinator (Korean Speaking)

Location:

Skokie, IL • Onsite with travel to local nursing homes

Pay:

$81K–$100K

Schedule:

Full-time, Monday–Friday (occasional travel as assigned)

You'll support smooth member onboarding and care continuity by bridging facility teams, NPs, and care coordination staff. You'll be the on-the-ground RN connecting data, people, and process.

What You'll Do

Support onboarding of new health plan members and facilities

Train nursing home staff on the clinical model and EMR use

Review facility EMRs to flag member care and transition needs

Facilitate specialist access and external appointments

Communicate updates between facilities, NPs, and the coordination team

Assist with prior authorizations and transitions of care

Help launch telehealth services in new facilities

Collect data for hospital reviews and quality meetings

Must-Haves

Active RN license in good standing

Associate Degree in Nursing (BSN preferred)

Fluent in

Korean

(spoken and written)

At least 2 years of RN experience

Strong EMR and Microsoft Office skills

Valid driver's license and proof of insurance

Able to travel locally as assigned

Nice to Have

Nursing home or managed care coordination background

Experience with member onboarding or care transition programs
