Conduct evaluations for patients in both inpatient and outpatient environments and create personalized therapy plans Implement physical therapy strategies with a focus on enhancing mobility, strength, balance, and overall function Work in partnership with nurses, physicians, case managers, and rehabilitation specialists to elevate patient outcomes Educate patients and their families about rehabilitation goals, discharge procedures, and home exercise protocols Maintain precise documentation of evaluations, therapy sessions, and patient progress in accordance with clinic and regulatory standards Engage in care team discussions and interdisciplinary meetings to promote coordinated patient care Education
Doctorate in Physical Therapy (DPT) or a comparable accredited degree Certifications
A valid state license to practice as a
Physical Therapist CPR certification is preferred but not mandatory Specialty certifications (such as in geriatrics or neurology) are valued but not required Skills
Relevant clinical experience is highly preferred Expertise in treating patients with various medical and mobility challenges Excellent communication and teamwork abilities Proficient in digital documentation systems for clinical record‑keeping Benefits
