The RN Designee/Service Coordinator Director plays a pivotal leadership role in overseeing and coordinating nursing services and care management within healthcare programs. This position ensures the delivery of high-quality, patient-centered care by supervising service coordination activities and acting as a clinical resource for nursing staff. The role involves strategic planning, compliance monitoring, and collaboration with interdisciplinary teams to optimize health outcomes for diverse patient populations. The director is responsible for maintaining regulatory standards and fostering continuous improvement in service delivery processes. Minimum Qualifications: Current and valid Registered Nurse (RN) license in the United States. Bachelor of Science in Nursing (BSN) degree or equivalent. Minimum of five years of clinical nursing experience, including at least two years in a supervisory or leadership role. Demonstrated knowledge of care coordination, case management, and healthcare regulations. Strong understanding of patient care standards, quality improvement processes, and healthcare compliance requirements. Preferred Qualifications:
Experience working within managed care or community health settings. Proficiency with electronic health records (EHR) systems and healthcare data analytics. Experience in program development, budgeting, and strategic planning. Responsibilities:
Lead and manage the service coordination team to ensure effective delivery of nursing and care management services. Develop, implement, and monitor care plans in collaboration with healthcare providers, patients, and families. Ensure compliance with federal, state, and organizational regulations and standards related to nursing and service coordination. Provide clinical guidance and support to nursing staff, including training, performance evaluation, and professional development.
