Schedule: M-F 8am to 4pm Assignment Details: Contract Weeks: 13 Guaranteed Hours: 37 Requirements: Licensure as a Physical Therapist Assistant Knowledge of APTA guidelines and federal/state regulations Roles & Responsibilities: Under the direct supervision of a physical therapist, carry out individualized treatment plans in compliance with regulations and standards of care. Assist in the development of care plans to promote optimal functional independence and safety. Provide skilled physical therapy treatment including restorative and maintenance techniques, along with patient/caregiver education. Coordinate patient care with other clinical team members to ensure optimal communication and quality of care. Monitor and report changes in patient conditions to the case manager in a timely manner. Maintain accurate clinical documentation to reflect care provided and outcomes achieved. Compliance / Notes: Must stay up-to-date on HHCAHPS standards and integrate them into visit protocols.
