Market CRM Representative
is a local, field‑based relationship role supporting a defined group of skilled nursing facilities within an assigned market. This flexible, part‑time contractor position serves as a consistent, trusted presence on behalf of Puzzle Healthcare—strengthening partnerships, identifying early concerns, and supporting continuity between leadership touchpoints.
This is a local, market‑specific 1099 independent contractor role with an expected commitment of 8–15 hours per month and no out‑of‑market travel required.
What You’ll Do
Serve as a consistent, in‑person presence across assigned skilled nursing facilities
Build strong rapport with administrators, clinical leaders, and front‑line staff
Reinforce Puzzle Healthcare’s value through regular check‑ins and relationship maintenance
Proactively assess facility satisfaction and engagement levels
Identify early signs of concern, disengagement, or operational risk and elevate appropriately to the Regional Director of Operations
Track and report leadership and key staff turnover, ensuring smooth continuity during transitions
Share high‑level performance data and program updates with facility partners
Act as a reliable communication bridge between facilities and regional operations leadership
Represent Puzzle Healthcare professionally across all facility interactions

Residency within or near the assigned market
