Shelter Housing Specialist

Shelter Housing Case Manager offers services in a shelter setting, integrating evidence-based practices with individuals who are experiencing homelessness. These individuals may have mental and/or physical health needs. Such services include conducting assessments, developing service plans, providing case management, increasing access to benefits/healthcare/mental healthcare and community integration. Shelter Case Managers will implement evidence-based practices such as Housing First, Harm Reduction, Motivational Interviewing and Trauma-Informed Care. Shelter Case Managers will work collaboratively with a range of community service providers to ensure participants are connected to appropriate housing options.

Primary Responsibilities

Create a personcentered, progress based length of stay goals in collaboration with the participant addressing housing goals.

Utilize HMIS for all documentation.

Provide case management services in the area of, but not limited to: money management, employment, linkage to community providers for problematic substance use, primary and mental health care, and all other services needed to assist participants in reaching their housing goals.

Meet with Participants as scheduled based on their goals.

Use evidence based practices in service delivery such as Motivational Interviewing, Harm Reduction, and Housing First practices.

Provide case management services in areas of employment, benefits establishments, and other services needed to assist participants in housing goals.

Mediate and advocate on behalf of the participant to help them obtain housing, health care benefits and entitlements.

Requirements

Must pass a level 2 background check

High School Diploma or equivalent

2 year’ social service experience. Preferred in homeless services or case management.

Self directed and able to work without much supervision.

Organizational and communication skills (written & oral)

Ability to interact with all persons in a professional manner encompassing behavior, dress, language, and attitude.

Basic computer skills: proficiency with GSuite, HMIS and Slack.
