Social Service Director

Responsibilities:
Develop and implement social service programs and services to meet the needs of our residents and their families.
Collaborate with interdisciplinary teams to design and deliver patient‑centered care plans that address the social, emotional, and psychological needs of our residents.
Lead and supervise a team of social workers, counselors, and other social service professionals.
Develop and manage budgets for social service programs and services.
Ensure compliance with regulatory requirements and industry standards for social service practices.
Foster strong relationships with community partners and stakeholders to enhance our social services and advocacy efforts.
Monitor and evaluate program outcomes, identifying opportunities for improvement and implementing necessary changes.
Requirements:
Minimum 2 years of experience in social work, case management in Skilled Nursing.
